Nottinghamshire Integrated weight loss and smoking cessation – ABL Health
Your Health, Your Way is the new service commissioned by Nottinghamshire County Council to support Nottinghamshire residents to get active, lose weight and quit smoking. The service will also provide support for falls prevention and family weight management.
While the service is now live, due to the current situation with Coronavirus, all weight management (with the exception of maternity services), physical activity and alcohol support is currently postponed to enable Notts County Council to re-prioritise staff and resources to support in their health protection work around the current coronavirus outbreak. Weight management and physical activity referrals can still be made and will be held on a waiting list until the services resume.

Our smoking cessation services will continue to be provided remotely via telephone and digital support. Nicotine Replacement Therapy will be posted out to clients. The Your Health, Your Way service will begin processing referrals for stop smoking support and maternity weight management from Monday 6th April, however online/electronic referrals can be made from today. The service will be accessible to all Nottinghamshire residents via self or third-party referral. We have a single online referral form for easy referral for clients and health professionals. Electronic referrals can be made via the F12 function on SystmOne. Self-referral can be completed via the app, website (www.yourhealthnotts.co.uk) or by calling our administration team on 0115 772 2515. Referrals can also be made direct from hospitals or via nhs.net email. All referrals processed within 1 working day (from Monday 6th April).
