Bassetlaw Place-Based Partnership

Bassetlaw Placed-Based Partnership has been put in place to deliver health and wellbeing for Bassetlaw by simpler, integrated, responsive and well understood services – enabling people to get the right support at the right time.   This will support local people to stay well in their own homes and communities so everyone can be better in Bassetlaw.

Bassetlaw PBP is a partnership of  BCVS, Bassetlaw District Council, NHS Nottinghamshire ICB, Doncaster and Bassetlaw Teaching Hospitals NHS Foundation Trust, Healthwatch Nottingham & Nottinghamshire, Nottinghamshire County Council, Nottinghamshire Healthcare NHS Foundation Trust and three Primary Care Networks.

Primary Care Networks will link staff from general practice, community-based services, hospitals, mental health services, social care and voluntary organisations to help deliver joined-up care .
