South Nottinghamshire ICP

Partnership of commissioners, health and care providers and local authorities working together to join up health and social care services across Broxtowe, Gedling, Rushcliffe and Hucknall (Ashfield).

By working better together, the aim is to enable people in South Nottinghamshire to live healthier lives and get the care and treatment they need, in the right place, at the right time.

The partnership is built around six Primary Care Networks (known as PCNs) which are groups of GP practices that work alongside other services in the community.  Each PCN is directed by a GP who works in that area.

The South Nottinghamshire ICP lead is John Brewin, Nottinghamshire Healthcare Foundation Trust
